Check-in for the tournament will be held at West Pines Soccer Park (350 SW 196th Ave, Pembroke Pines, FL 33029, View Google™ Map) on Friday, October 22nd, 2010 from 6:00 pm - 9:00 pm for the GIRLS and Friday, October 29th, 2010 from 6:00 pm - 9:00 pm for the BOYS. Please bring the following documents to check-in:
- 5 FYSA State Generated Roster, AYSO printed roster or US Club Soccer Generated Roster (no other roster formats will be accepted)
- Original Guest Player Form(s), if needed (plus 4 copies)
- Laminated Player/Coach Passes (must match roster type)
- Notarized Medical Releases (one for each player, the form can be downloaded below)
There are a few teams who have Friday night games. If you have a Friday night game at another park, you will be able to check-in at that park before your game. If you have any questions, please contact kickoff@westpinesunited.org.
